PLAYER & COACH EVALUATION FORMS
Instructions/Process:
1. Choose the VERSION of evaluation form best suited for you.
2. Complete the evaluation. Complete one evaluation for each player on your team.
3. Spell YOUR NAME correct (or the evaluation will not track to your team).
4. Request your evaluations. Contact Caitlyn or Sierra, your advocate, once you have a completed a full team set.
5. Caitlyn or Sierra will send you in one email each of your individual evaluations and in a separate email your team analytics page.
6. Download the individual evaluations on your computer.
7. Rename each individual evaluation file with the corresponding player name.
8. Email/Disperse your evaluations to your players.

NOTE: It's important that you spell YOUR NAME correctly at the top of the form. This is how the forms will be "sorted". We will receive approximately 1,000 so you can help us help you by being meticulous.

VIRTUAL TEAM BUILDING ACTIVITIES - The resources below may prompt ideas for even better and more awesome virtual team building activities. Be creative. Feel free to modify for your team. A good point is that when hosting your first "Conf Call" or "Video Chat", it's a good idea to ask each player how their Spring Break was, what they did, etc.... Let the kids "be a team" again, and then get into the activity that will help prompt further connections, engagement, and growth.
