I intend to apply for financial assistance through the WSA Soccer Financial Assistance screening process.  I understand that the process will ONLY be offered ONLINE through a virtual set up and not IN person.

I understand that this process may require producing documentation to validate my family's need.  I understand assistance at the level I request is not guaranteed and subject to review, as well as available funding. I also understand WSA's mission involves finding a way for every kid to play and WSA will exhaust all measures to assist where assistance is warranted. I promise to be honest and forthright in the process.  Likewise, by checking this box I acknowledge commitment to the terms of the agreement, with the exception of the financial commitments spoken and detailed herein contingent and pending financial assistance review, and commitment to a subsequent revised financial agreement.
